The Wisconsin Veteran Self-Directed Program allows eligible veterans and their families choose the services and supports they need to safely and successfully live at home. Services may include:
• Personal care
• Supportive home care
• Adult day care
• Assistive technology
• Home-delivered meals
• Respite care

How Does It Work?

VA staff identify and refer the veteran to GWAAR and a Care Consultant is assigned. GWAAR’s Care Consultants provide support to empower veterans to manage their own care—from plan to payments.

Veterans may be eligible if enrolled in primary care with the Captain James A. Lovell Federal Health Care Center, the Tomah VA Medical Center, or the Clement J. Zablocki VA Medical Center. Contact your VA primary care provider to learn more.
